One of our Clients team was seriously injured on site. The incident was reportable under RIDDOR and we worked with our Client and their insurers to ensure a full investigation was undertaken. Although our Client called us immediately post incident, which was on a Saturday, Pearl Safety swung into action and coordinated the initial actions to make the area safe and preserve any evidence. Our investigation was complex and hampered by third party suppliers who refused to discuss the incident, however by utilising contracted forensic specialists and working with the site management, we gathered the evidence required to identify the root cause.
Our Client subsequently dismissed the specialist provider and sourced a replacement company. This case involved a deep dive analysis of procedures and human behaviours. All aspects of the works and plant management had been complied with and although the specialist provision had been assessed by a robust PQQ process, they failed to deliver their contracted works to our Client.
